Application should include:

1. Name of Congress chair.
2. Primary contact person: It would be best if this person is conference chair and we must have a primary contact person who can respond to e-mails in English.
3. Proposed names of members of the scientific committee.
4. Description of meeting site
   1. Documentation from meeting site that they can host event
   2. Floor plans, number of attendees site can hold
5. Description of Hotel accommodations
   1. Names and Location of hotels (Distance to congress site), and number of rooms available.
6. Overall meeting Theme
7. Anticipated dates for meeting
8. Anticipated number of attendees
9. Likely funding: source, amounts
10. Agreement to host congress under “guidelines for ISPD congress” document.
    1. Description of financial sharing
    2. Agreement to include persons from ISPD council on scientific committee
    3. Agreement to develop agenda and enlist speakers for the congress in a timely manner
    4. Agreement to regularly attend ISPD council meetings if selected in order to report on the progress of the congress.
